This Licensed Practical Nurse (LPN) position provides Case Management to clients living in the Airdrie area. This position requires advanced critical thinking abilities and a wide-ranging interventions skill set & experience. In collaboration with the with the interdisciplinary team, the Licensed Practical Nurse is responsible to assess, plan, implement and evaluate the needs of Home Care clients and their caregivers / families. The Licensed Practical Nurse is responsible for their nursing practice as defined by the College of Licensed Practical Nurses of Alberta (CLPNA). There is also an expectation to be a mobile worker. Possession of a valid drivers’ license and access to a motor vehicle in good repair for work related purposes is a requirement of this position. You will be responsible to carry and move equipment, use and operate a laptop computer and mobile phone, and for manual handling (lifting up to 30 lbs., lowering, carrying, pushing and pulling).

As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
Required Qualifications :
Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications : Preferred Qualifications :
Recent work experience in a community-based setting, Home Care experience preferred. Experience working with complex chronic disease(s) and Palliative clients. Experience in Case management practice and completion of the McMaster AHS Case Management education. Advanced wound care certificate. Experience in Home Parenteral Therapy (HPT), wound, skin and ostomy care, palliative support and client teaching. Completion of a post-basic Medication Administration Course for LPNs.
