ABOUT US

Siloam Mission is a Christian-based non-profit service organization for Manitobans who have experienced homelessness and who struggle with mental health issues physical and cognitive disabilities addictions or trauma. Founded in 1987 the mission provides emergency shelter meals and clothing supportive housing and a variety of healing support and recovery services to help people transform their own lives and reach their full potential.

JOB SUMMARY

Siloam Missions Medical Transitional Care Unit (MTCU) is a compassionate 32-bed facility supporting individuals experiencing homelessness who have medical needs that do not require hospitalization. The MTCU provides time-limited transitional housing integrated with essential medical and social supports to improve health and housing outcomes.

Guided by person-centered strengths-based trauma-informed practice and rooted in harm reduction and Housing First principles the MTCU operates through a holistic and multidisciplinary approach.

The Health Care Attendant (HCA) supports residents of the MTCU to live with dignity while improving their health safety and overall wellbeing. Working collaboratively with the interdisciplinary MTCU team and external partners the Health Care Attendant provides direct personal care supports activities of daily living and contributes to a safe respectful and healing environment.

KEY DUTIES & RESPONSIBILITIES

Resident Care & Daily Living Support

  • Support residents with activities of daily living (ADLs) including bathing dressing toileting feeding oral hygiene lifting moving repositioning or assisting residents on or off beds toilets wheelchairs a manner that promotes dignity choice and independence
  • Respond to residents physical emotional and psychosocial needs using a trauma-informed strengths-based approach
  • Assist residents with routine household tasks such as laundry and maintaining personal living spaces
  • Collect handle and dispose of soiled linens trash infectious or hazardous waste according to applicable policies laws and environmental standards
  • Sort and distribute residents mail
  • Support meal service by assisting with distribution of prepared meals and collection of dishes ensuring residents receive pre-approved meals according to their dietary requirements
  • Organize daily work activities according to resident needs assigned tasks and unit priorities

Delegated Clinical & Health-Related Tasks

  • With appropriate education training and authorization and in accordance with regional policies operational directives and delegation processes perform assigned care tasks which may include:
  • Medication assistance (oral topical transdermal inhaled eye/ear/nasal suppositories and subcutaneous injections)
  • Blood glucose monitoring
  • Ostomy care
  • Adjusting oxygen flow rates
  • Respond to emergency situations and collaborate with staff security and external agencies during incidents to ensure safety
  • Follow clinical direction and medical orders as assigned and within scope of practice

Communication Documentation & Safety

  • Communicate respectfully empathetically and effectively with residents team members and external partners ensuring dignity is maintained
  • Observe document and report resident care provided and any changes in condition or behaviour in a timely and accurate manner
  • Share relevant information with the MTCU team to support care planning using approved documentation systems and processes
  • Report safety concerns supply needs maintenance issues and critical incidents in accordance with organizational policies
  • Maintain confidentiality and comply with all privacy safety and ethical standards and legislative standards

Teamwork Quality & Professional Practice

  • Establish and maintain positive professional relationships with residents staff external partners and volunteers
  • Participate in team meetings handovers and quality improvement activities as assigned
  • Perform duties in a manner that reflects Siloam Missions values of Holistic Equity. Altruism Relationship Truth and Reconciliation
  • Engage in ongoing learning training and professional development
  • Perform other related duties as assigned by management

EDUCATION & EXPERIENCE

  • Completion of a recognized Health Care Aide program Personal Support Worker program or other relevant education/training preferred
  • Training in NVCI ASIST Mental Health First Aid and Standard First Aid WHMIS PHIA preferred
  • Recent and relevant experience providing direct participant or patient care required
  • Satisfactory Criminal Record Check with Vulnerable Sector Search and Adult Abuse Registry Check required
  • Proficiency in medical terminology an asset
  • Experience with Electronic Medical Records (EMR) software an asset
  • Lived experience is an asset

  • Understanding of colonization and its ongoing impacts on Indigenous peoples

  • Consistent application of harm reduction and Housing First principles
  • Recognizing the challenges community members face with lived/living experience
  • Ability to work effectively within an interdisciplinary team
  • Ability to stay calm and professional during high-stress or crisis situations

RELATIONSHIP

  • Reports to the Manager of Medical Transitional Care Unit
  • Works closely with Clinical staff in MTCU
  • Daily interaction with residents team members and volunteers
  • Participation in staff meetings brainstorming sessions and team building activities

WORKING CONDITION

  • Hours of Work:
    • 6:00 a.m. 2:30 p.m. (Mon-Fri; Wed-Sun; Fri-Tue)
    • 2:00 p.m. 10:30 p.m. (Mon-Fri; Tue-Sat; Thu-Mon; Sat-Wed) shift premium
    • 10:00 p.m. 6:30 a.m. (Mon-Fri; Wed-Sun; Fri-Tue) shift premium
  • May be required to lift up to 50lbs repetitive motions (bending twisting lifting etc.)
  • Exposure to blood and body fluids infectious diseases allergens (ie. residual smoke) noise and emotionally demanding situations
  • Indigenous ceremonies and practices including smudging will occur
  • Hazardous conditions could potentially include unpredictable or risky behaviours from community members;

WORKPLACE SAFETY AND HEALTH

The incumbent contributes to making the organization safe for clients and staff and recognizes the importance of reporting unsafe situations and participating in follow-up reviews as a learning opportunity.

  • Provides a safe environment by ensuring adherence to Workplace Safety and Health Regulations and Policies Infection Control Guidelines WHMIS and Safe Work Procedures
  • Immediately investigates and recommends corrective action on any unsafe acts work conditions incidents near misses injuries or illnesses
  • Demonstrates understanding of role and responsibilities in fire prevention and disaster preparedness and participates in safety and health training programs including the facilitys Fire Disaster and Evacuation Plan
  • Supports an environment which avoids prevents and corrects all activities or actions which may result in an adverse outcome in the delivery of services. Community Member safety is a standing item for all individual and departmental meetings

Salary/Wage: $41768 - $56509
