- Provides sound clinical knowledge and assessment skills, acts as clinical expert and resource and provides comprehensive, quality client / family centred health care services from admission, through transfers and discharge
- Acts as a case manager to clients and their families
- Connects clients and their caregivers / families to community resources that will help the client to live well in the community and manage their illness
- Plans for, delivers and evaluates nursing interventions, including medications, psychoeducation and intervention, as well as treatments which fall within the RN’s scope of practice
- Participates actively in the teaching programs for students including, but not limited to nurses and nursing students
- Actively participates in program development and evaluation
- Able to drive to visit client’s and families in their homes within the GPCS catchment area
Qualifications include but are not limited to :
Current registration, in good standing, from the College of Nurses of Ontario (‘CNO’) is requiredCurrent Basic Cardiac Life Support (‘BCLS’) certificate, Level C - Automated External Device (‘AED’) is requiredBScN preferredDemonstrated ability to take professional accountability for leading, organizing, providing and delegating care activities for clients and familiesPsychogeriatric experience, i.e. mental health assessment and intervention and working with BPSD preferredKnowledge of community resources for clients and familiesDemonstrated ability to work independently and within an interdisciplinary teamExcellent interpersonal, communication and problem-solving skillsBasic computer skills – Meditech, Microsoft Word, Microsoft 365, and Excel preferredAdditional Benefits :
