The Opportunity

Reporting to the Director of Care, the Licensed Practical Nurse (LPN) is responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care.

Key Responsibilities

• Work as a member of the inter-professional team, as described by our Professional Practice Model, applying a person-centered approach to care.
• Complete and ensure resident clinical care assessments, care/service plans, and documentation are accurate, up to date, and communicated as required.
• Implement and evaluate the effectiveness of care/service plans, making changes as needed.
• Assist with scheduling resident care conferences and participate when applicable.
• Work closely with the Director of Care to facilitate resident move-ins, move-outs, and transfers.
• Provide direct resident care, administer medication/treatments, and document care in compliance with regulatory and Resident Care standards, policies, and procedures.
• Process orders, safely store drugs and narcotics, and administer them per regulatory requirements, scope of practice, and policies and procedures.
• Facilitate and provide resident/family education.
• Plan, coordinate, manage, and evaluate resident care plans in collaboration with the resident/substitute decision maker, physicians, nursing and interdisciplinary teams, and contracted service providers.
• Coordinate nursing team assignments to ensure efficient and timely service delivery.
• Respond to and manage significant changes in resident conditions or medical emergencies, providing emergency first aid as needed.
• Teach, delegate, and direct unregulated care providers.
• Coordinate emergency response in the absence of the Director of Care or Director of Health & Wellness/Executive Director.
• Address resident and family concerns, ensuring appropriate action is taken or escalating issues to the Director of Care, Director of Health & Wellness/Executive Director, or other departments.
• Assist the Director of Care or Director of Health & Wellness in coordinating, implementing, and monitoring Infection Prevention and Control, Health and Safety, Clinical Programs, and Quality/Safety Plans for the Nursing Department.
• Work in a position of trust with vulnerable adults (residents).
• Uphold and promote the organization's values and philosophy, particularly regarding ethics, morality, and integrity, as outlined in the Code of Conduct.
• Perform other duties as assigned.

Qualifications

• Current registration as a Licensed Practical Nurse in the applicable Province/State.
• Experience working in a senior’s environment and with unregulated care providers preferred.
• Education and experience in gerontology, rehabilitation, community, complex, or long-term care an asset.
• Strong written and oral communication, critical thinking, care coordination, time management, and customer focus skills required.
• Demonstrated sound clinical knowledge.
• Current Basic Cardiac Life Support and First Aid certification required.
• Computer proficiency and experience with Microsoft Office applications.
• Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one.
