McMan is recruiting foran Evening Case Manager to join our teamprovidinga housing first and harm reduction approach; the Case Manager will be responsible for assisting residents in maintaining their housing assist accessing financial supports supporting their education / employment goals and any other areas of support that is required to help them successfully transition into independence. The Case Manager will work alongside our Transition Specialists by being a member of the residents team that is planning around the needs for their successful transition into the community.

Some of the key aspects of this role include :

• Complete case management responsibilities that include assessments goal planning case consultationmonitoringand evaluation of the goals to meet the clients needs.
• Educate and support individuals to increase skills that promote greater self-efficacy.
• Provide individual instruction modeling and support of basic life skills (ie : health nutrition home cleanliness safety).
• Monitor and support individuals through a harm reduction approach to ensure living conditions are safe and sanitary.
• Implement integrated service delivery from the point of referral to program completion and transition for individuals accessing the program.

Salary Range : $49768.00 -$54931.00annually
