Nurse Practitioner (Temporary Part-Time)
1-year term opportunity with possibility of extension
KEY RESPONSIBILITIES
Reporting to the Medical Director, the Nurse Practitioner (NP) will be responsible for providing medical care to clients in the Home Visiting Hub (HVH) program. The NP will liaise with the HVH Manager, the HVH nurses and HVH physicians supporting continuity of care and 24 / 7 prescriber coverage. The NP is supported by the Home Visiting Hub Manager and nursing staff.
The NP will provide home visits to 15-20 (part time) or 20-30 (full time) clients in the community who are unattached and require palliative care. NP’s are expected to be available to HVH patients, for in-person, or virtual visits, Monday through Friday, 8am-4pm, adjusted accordingly based on FTE.
Responsibilities Include :
- Regular home / virtual visits to patients remaining at home for part or all end-of-life care.
- Symptom monitoring and management, including prescribing appropriate medications and follow up.
- Guidance and goals of care discussions with patients and family through end of life.
- Attend Home Visiting Hub (HVH) Rapid rounds and HVH Handover rounds as scheduled either in person or virtually. Current schedule is Monday and Wednesday mornings at 0800-0900 and HVH Handover rounds Thursday afternoons at 1500-1600. Schedule is subject to change.
- Liaising with nurses and other health care professionals / programs to ensure care needs are being met at home (MIH, Home care, PCCT).
- Referrals to appropriate health care professionals / services (ATOP, PCCT, TPCU).
- Providing guidance around hospice care and services within the Edmonton zone.
- Participation in the rotation to provide on call services after hours for Home Visiting Hub. Coverage will be home-call exclusively.
- Provision of Hospice coverage on as needed basis and if scheduling permits.
- May be required in certain scenarios to participate in an on call schedule to ensure continuity of care, including overnight and weekend coverage.
- Ensure accurate, objective, and timely clinical documentation.
ESSENTIAL QUALIFICATIONS
Minimum of 3-5 years’ experience providing Hospice / Palliative End-of-Life Care.Exceptional interpersonal and communication skills and the ability to work effectively with a wide range of individuals and stakeholders (interdisciplinary team members, AHS Case Managers, Consultation Team, clients, families).Community experience is considered an asset.Must be licensed and in good standing with the College of Registered Nurses of Alberta.Completion of LEAP (or expected completion within four (4) months of start date), required.Must have a valid Alberta Driver’s License and access to a vehicle for travel required within Edmonton and surrounding area.TO APPLY
