The Grande Prairie Primary Care Network, in beautiful Grande Prairie, Fairview, and Grimshaw, Alberta, was formed in 2010 to work with local Grande Prairie primary care physicians in the delivery of primary healthcare services. The PCN provides innovative and collaborative initiatives that improve communication, increase coordination of services, and enhance medical clinic practices to better meet the needs of the community.
The Social Worker’s primary function is to work with patients to determine, advocate for, and refer to appropriate community resources and programs. The Social worker will collaborate with Primary Care Physicians, their clinical staff and other members of the Primary Care Support Program and Persistent Pain Program to deliver support using a patient-centred approach.
This is a 0.8 FTE (30hrs per week) permanent position.
Major Duties & Responsibilities
- Implements a comprehensive patient assessment process
- Facilitates goal development with patient and works towards completion of identified goals
- Works with patients to build capacity to self-manage their conditions and circumstances
- Ensures that all relevant data and history is available and entered into the Electronic Medical Record (EMR) in accordance with practice standards and organizational policy
- Communicates with team members regarding patients’ concerns, needs and progress as necessary
- Participates in case consultation with clinical supervisor
- Undertakes community visits related to patient needs
- Collaborates with community agencies as required
- Develops content and presentations for patient and community education and events
- Facilitates education sessions for staff, patients, and community agencies
- Supports PCN community communication efforts (i.e. Newsletter, social media, interagency mailing lists, etc.)
- Participates in community events to promote PCN services
- Identifies appropriate agency contacts for collaboration purposes and engages in regular contact as required
- Attends and participates in interagency forums and meetings related to role as directed by Program Manager
- Makes appropriate referrals to other services and resources
- Maintains current knowledge and skills in social work practice
- Maintains knowledge of and adherence to organizational policies and procedures
- Adheres to Canadian Association of Social Workers’ Code of Ethics and Guidelines for Ethical Practice and Alberta College of Social Workers’ Standards of Practice
- Maintains professional requirements and actively participates in ongoing professional development
- Actively participates in program development and quality improvement activities
- Offer training support for other team members and assists in their professional development
- Acts as a resource to other PCN staff
