What Makes this Position so Exciting?
In this exciting staffing model, HISH CSWs will work 12 hour shifts and have the opportunity to be a respected member of a larger healthcare team striving to provide high quality patient care and positive transition experience for both the patient and their caregivers.
The hours of work will be 7, 12-hour shifts within a 2-week rotation inclusive of weekends :
Two-week Rotation
Mon / Tues / Sat / Sun
2000 -0800hrs
Wed / Thurs / Fri
2000 - 0800 hrs
We are looking for Community Support Workers (CSWs) to help us bring wrap-around patient-centered care to the Durham Region .

- Valid certificate as a Personal Support Worker, Internationally Trained Nurse, Health Care Aid, Developmental Support Worker, Home Support Worker, or enrollment in a Nursing program
- To protect our patients, we require current vulnerable sector check screening, a clear background check
New Ontario PSW grads may be eligible to
receive a $10,000 bonusFrom an approved PSW program in Ontario (on or after April 1, 2024)in exchange for a commitment to work with us, position for 12 months, payable at 6 months and 12 months must meet HFO eligibility and HFO eligibility statusand an additional $10,000 relocation bonus
when relocating to rural, remote or northern area and committing to working with us for 12 months, payable at start and 6 monthsWhat The Role Involves
Contributing as a valued member of the interdisciplinary care teamAssisting with daily living activities, such as personal care (rise and retire), assistance with household tasks, respite care, medication assistance, special functionsPromoting a safe environment by demonstrating proper body mechanics, safe transfer techniques, and home safetyProviding cognitive, social and / or emotional support to the patientPreparing nutritious, varied and economical meals and snacks, including special diets, as determined by the care plan, using proper food preparation, storage, and hygiene practicesProviding patient status reports and participate in patient / family care planning meetingsWhat You Bring

In addition to providing home-based health care, CarePartners also serves the community through clinics, transitional care units, and provides relief in retirement homes and shared care settings. Through our Community Nursing Services outreach program, we’ve been organizing staff-led medical care and clinics in countries with poor access to health care since 2009.
