Case Manager – East Assertive Community Treatment Team (ACTT) – Full-Time Permanent

The Role and Opportunity

The Canadian Mental Health Association (CMHA) – Toronto is looking for a Case Manager for our East Assertive Community Treatment Teams (ACTT). Based at the Markham site, the multidisciplinary ACT Team provides community-based support, rehabilitation, and treatment to individuals living with mental illness. The team focuses on promoting stabilization, community integration, recovery, and quality of life through client‑centered services delivered in the community.

Responsibilities

  • Assessment & Service Planning : Conduct comprehensive assessments to understand clients’ needs, strengths, and goals. Develop and implement individualized service plans in collaboration with clients and their support networks.
  • Independent Living Support : Teach life skills such as hygiene, nutrition, money management, and household management to foster independence.
  • Health Promotion : Support clients with medication management, access to primary healthcare, and coping strategies for daily functioning.
  • Crisis Prevention & Intervention : Develop crisis plans, monitor clinical changes, and intervene to ensure safety and stability. Provide on‑call coverage on a rotational basis.
  • Advocacy & Community Integration : Advocate for clients with various service providers (e.g., healthcare, courts, social services) and help them navigate community resources.
  • Supportive Counselling : Offer therapeutic support tailored to client needs, including CBT, DBT, MI, and solution‑focused approaches.
  • Employment & Social Skills Development : Assist clients in exploring employment opportunities and developing social and recreational skills.

Experience & Education Required

  • Post‑secondary degree / diploma in social services or a related field.
  • Minimum of 3 years in direct service delivery to adults with psychiatric disabilities, preferably in a community mental health setting.
  • Strong communication, interpersonal, and organizational skills, with the ability to work independently and as part of a multidisciplinary team.
  • Commitment to anti‑racism, non‑homophobia, and recovery‑oriented principles.
  • Valid Ontario driver’s license and access to a vehicle.
  • The role involves a varying schedule including weekdays, weekends, holidays, and occasional evenings as needed to meet operational demands.

    Hours of Work : 35 per week
