Job description

The Health Care Assistant (HCA) provides caring, respectful, and culturally safe support to Esquimalt Nation members so they can live as independently and comfortably as possible in their own homes. Under the direction of the Manager of Health and with guidance from the Home and Community Care Nurses, the HCA helps community members with personal care, meals, mobility, and light household tasks. The role includes checking on clients’ health and well-being, encouraging social and community involvement, and working closely with families and other members of the Health team. The HCA follows health and safety procedures, reports any concerns or changes in a client’s condition, and supports care that honours each person’s dignity, culture, and connection to community.

  • Helps clients with personal care such as bathing, dressing, grooming, and hygiene, in ways that respect their privacy, dignity, and cultural preferences.
  • Supports clients with moving safely, including getting in and out of bed, using mobility aids, and staying comfortable and active.
  • Prepares and helps with meals and feeding, making sure clients’ food preferences and nutritional needs are respected.
  • Checks and records vital signs like temperature, pulse, and weight, and applies creams, ointments, or simple dressings as directed by the Community Care Nurse.
  • Notices and reports any changes in a client’s health, mood, or overall well-being to the Community Care Nurse or Health Manager.
  • Reminds clients to take their medications and lets the nurse know if there are any concerns about how medications are being used.
  • Helps set up and use mobility or comfort equipment, and suggests items that could improve safety or independence.
  • Encourages clients to take part in family, cultural, and community activities that support social connection and well‑being.
  • Performs light housekeeping tasks like sweeping, vacuuming, laundry, and tidying to keep the home clean, safe, and comfortable.
  • Drives or accompanies clients to appointments, community events, or essential errands such as shopping for groceries or personal items.
  • Records care activities, observations, and updates clearly and accurately.
  • Follows safety, confidentiality, and infection control procedures to protect clients, families, and staff.
  • Reports any safety concerns, suspected abuse, or risks in a client’s home or health condition right away.
  • Works together with families, nurses, Elders, and other service providers to make sure care is well‑coordinated and respectful.
  • Speaks up for clients’ needs and preferences, ensuring care reflects their cultural values, dignity, and connection to community.
