Job Description

This is a unique opportunity to be part of a tight-knit, collaborative team from across the health sector that will deliver the next generation of palliative care in Mississauga, providing holistic proactive, seamless care and support to patients, families and caregivers – enhancing quality of life, comfort, dignity and security in patients’ end of life journey. As an integral member of the interdisciplinary care team, the Occupational Therapist (OT), Palliative Care works in collaboration with the care team, client and family, integrating a palliative approach. Specifically, the OT is responsible for conducting assessments, developing and delivering treatment plans focused on achieving the goals of care including promoting independence, enhancing comfort, and delivering education to support daily functioning and safety at home. Care will be provided both in-person and through virtual consultations to maximize our ability to provide services in the community.

  • Works under the supervision of the Program Director
  • Collaborates with the interdisciplinary team to create personalized care plans based on the patient’s unique needs and goals of care.
  • Participates in clinical huddles and case conferences with team members such as physicians, nurses, social workers, speech therapists and physiotherapists
  • Conducts thorough assessments of cognitive, functional, and psychosocial abilities to facilitate effective communication and engagement for individuals with life-limiting conditions
  • Identifies and assists individuals in acquiring appropriate assistive technology. Modifies existing technologies to meet individual needs, ensuring optimal usage
  • Effectively educates caregivers in necessary skills for assisting with personal care and safe transfer techniques, enhancing the care environment for patients and their families
  • Maintains accurate and detailed documentation, including assessments, treatment plans, progress notes and discharge summaries

Qualifications

  • Master’s degree in Occupational Therapy
  • Registered in good standing with the College of Occupational Therapists of Ontario
  • 3-5 years of experience working as an Occupational Therapist preferred
  • Successful completion of any of the following is considered an asset : Learning Essentials Approaches to Palliative and End of Life Care (LEAP); orFundamentals of Palliative CareThe Comprehensive Advanced Palliative Care Education (CAPCE)Experience working in a community setting
  • Valid Ontario driver's license and a reliable vehicle
  • Ability to work autonomously and as part of an integrated health care team, with an emphasis on collaborative care planning
  • Strong understanding of the clinical presentation and disease trajectories of life-limiting conditions.
  • Proficiency in conducting assessments, interpreting findings, and formulating evidence-based treatment plans.
  • Familiarity with assistive devices, adaptive equipment, and environmental modifications
  • Strong clinical skills and a passion for improving the quality of life for patients and their families
  • Ability to write detailed occupational therapy consultation reports
  • Proficiency in computer software programs (, Microsoft Office) and virtual communication platforms (, Microsoft Teams, Zoom, Go To Meeting)
