Case Management Worker – Non 24 Hour Programs

Bargaining Unit Position)

Hours of Work : 5 days per week, normally 9am to 5pm, with one evening per week and some flexibility required

Starting Date : ASAP

Salary : $28.76 / hour, as per Collective Agreement

This position supports clients in their housing by assessing and supporting their needs and adhering to the mission statement and policies & procedures of Homes First Society.

Job Description :

  • facilitate tenant movement from shelters to secure long term permanent housing
  • assist tenants in maintaining housing
  • work with site staff to develop outreach and needs assessment criteria
  • knowledge of rental legislation in Ontario and Hostels Guidelines and Procedures
  • knowledge of current housing and support agency providers appropriate to our tenants and residents
  • ability to understand and assess suitability of potential landlords and / or residences
  • identify current issues, determine supports necessary, refer and support clients in preparation to entry level / transitional housing, and with referring worker, support move to site
  • confer with front line support staff at site to transfer information from case file to staff team
  • use case management as a tool to support residents in meeting their goals
  • provide support including work readiness, basic life management skills, anger management, addiction counseling, assertive communication, setting goals, giving and receiving feedback, self-esteem, conflict resolution, problem solving and managing change
  • provide support to clients who are facing issues of poverty, mental health, addictions, abuse, social isolation and more.
  • apply knowledge of group processes and dynamics
  • use excellent organizational and time management skills
  • work independently and as part of a team
  • communicate effectively in English, both verbally and in writing.

Qualifications :

  • post-secondary education or equivalent.
  • several years’ experience working with people who are homeless, under-housed and who are hard to house.
  • knowledge of tenant population, diversity issues, anti-discrimination practices and systemic reasons for homelessness.
  • knowledge of rental legislation in Ontario
  • excellent interpersonal skills.
  • knowledge of relevant laws, standards and guidelines.
  • knowledge of resources and services available in the community.
  • excellent interpersonal, conflict resolution and organization skills.
  • certificates in First Aid, CPR, Crisis Prevention and Intervention an asset
  • Certificate of Completion of Homelessness Learning Hub’s Harm Reduction for the Homelessness Sector considered an asset
  • ability to work occasional evenings
  • Working in an environment where there may be smoke is required.
