The Nurse Practitioner (NP) works directly with the Child and Youth Mental Health program (12-17yr) at LHSC and is responsible to provide comprehensive addiction care using advanced assessment skills and a sound knowledge of clinical therapeutics. The NP provides direct and indirect services to support patient care in collaboration with various inter-professional teams in order to support addictions needs. The NP will work closely with community partners to ensure a smooth transition to community-based addiction resources upon discharge.

WHAT WE'RE OFFERING:

  • What you'll be making - Starting rate for this role is $65.69 per hour (Level 1 of the Nurse Practitioner Grid);
    • Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8, $70.87 per hour
    • Internal applicants will be placed on the grid based on their seniority
  • When you'll work - 70 hours over two-week period; Monday-Friday 8:30am-4:30pm
  • What your benefits will be - After a 90-day waiting period will be enrolled in extended healthcare benefits
  • Your pension plan – Immediately enrolled in the Healthcare of Ontario Pension Plan (HOOPP)


HOW YOU WILL BE MAKING A DIFFERENCE:

Primary Responsibilities
  • Retain clinical competence through relevant evidence-based research and legislation
  • Demonstrate ability, commitment to and knowledge of community addictions and mental health care
  • Provide non-urgent, health care services within the NP's full scope of practice to clients who encounter barriers to accessing addiction care. Consults with collaborative physicians for clients who have unstable/complex health conditions.
  • Participate in the development of and work under current medical directives for the program
  • Provide and evaluate holistic patient care based on advanced nursing and medical management knowledge and skills.
  • Exhibit knowledge of evidence-informed guidelines, legislation, regulation and organizational policies when prescribing controlled drugs/substances (CDS).
  • Demonstrate a understanding of best practices related to the use of CDS and other pharmacological and non-pharmacological therapeutic options.
  • Demonstrate excellent communication, decision-making, problem solving, conflict management and time management skills.
  • Demonstrate ability to work effectively in an interdisciplinary team environment.
  • Perform comprehensive care using advanced health assessment, physical examination, therapeutic management, consultation, collaboration and referrals in accordance to the Practice Standard: Nurse Practitioner.
  • Maintain client records for accuracy and confidentiality.
  • Support the health needs of individuals with a history of addictions, mental health and other chronic illnesses and refer to the specialists and other community agencies.
  • Demonstrate ability to deal effectively with sensitive issues with clients and families using a harm-reduction and trauma-informed approach.
  • Determine the need for follow-up appointments and assist with transfer of care.
  • Work in partnership with primary care providers, community agencies and programs.
  • Coordinate with service provision with other health care partners in the development of community care plans and wrap-around services.
  • Proficient in computer skills and experience with electronic medical record systems (ie. PS Suites).
Teamwork:
  • Participate as a team member in the day to day hub operations
  • Actively engage in learning opportunities, events, meetings and team building activities scheduled by the organization for staff members
  • Act as a resource to other staff and community partners; seek out and listen to input from other staff and community partners.
  • Provide support and encouragement to other staff as needed (eg. assistance with challenging participants/clients, coverage for vacation, sick time, share responsibility for workloads).
  • Be supportive of, and an active participant in, the constant evolution and growth of the organization.
  • Promote a safe and healthy work environment by identifying potential hazards in the work environment and taking appropriate action by notifying a member of the JHSC or Management rep of any potential health and safety concerns, risks or near misses.

WHAT YOU'LL NEED TO APPLY:

Police Vulnerable Sector Check Requirements

Successful candidates must have a Police Vulnerable Sector Check completed in the last six months. Interested applicants who reside in the City of London and are in need of a Police Vulnerable Sector Check (PVSC) are encouraged to apply for one as soon as possible as PVSCs in the City of London are currently taking approximately 2 weeks to be processed and offers of employment cannot be made without receipt of a PVSC.

We require the following qualifications:

  • Current registration with the College of Nurses of Ontario as a Registered Nurse (in the Extended Class)
  • Successful completion of the Prescribing Narcotics and Controlled Substances course
  • Current certificate in cardiopulmonary resuscitation (CPR)
  • Valid Driver’s License, adequate insurance and access to a vehicle to carry out job duties
  • Current, clear Police Vulnerable Sector Check (PVSC)
  • Demonstrated ability to attend work regularly


We prefer the following qualifications:

  • Minimum 2-3 years’ experience of community health care experience
  • Demonstrated ability to work with clients who live with multiple challenges related to marginalization, mental illness, substance use, complex health challenges, street level sex work, poverty, homelessness, and trauma
  • Extensive knowledge of community resources, service & support programs, and barriers clients face in accessing same
  • Working knowledge of a variety of treatment, intervention, and support strategies
  • Computer literacy
  • Master's Degree
  • Suboxone on-line education program
  • Experience in addiction medicine and harm reduction
