Client Care Management
Performs assessments in the community (Ensures standards of care, compliance with College of Nurses standards, state / provincial regulations and standards of careDevelops standards that ensure safe and effective services to clients and families;Establishes care plan, completes and maintains related records and documentationEnsures a high standard of customer service is delivered to clients from intake to client conversion, case setup- up and delivery of service;Ensures that the level of care established for clients is safe and appropriate based on client and home assessmentDirects the implementation of service goals and objectivesEnsures the evaluation of client care servicesAchieves / exceeds client satisfaction, investigates issues, and follows up promptly;Qualifications
Registered with the College of Nurses of Ontario (CNO) in good standingExperience in doing assessments in Home and Community Care ( 1-2) yearsCurrent BCLS certification (Level C)Demonstrated clinical assessment and patient / family teaching skillsProven excellent interpersonal skills and effective oral / written communication skillsDemonstrated excellent organizational, time management, prioritization problem-solving, and conflict-resolution skillsDemonstrated planning and coordination abilitiesProven ability to work effectively as part of a teamDemonstrated ability to work with minimal supervisionRequirements :
Clear & updated Vulnerable Sector Check (within the last six months)Valid Driver’s License & reliable carUp-to-date TB test within the last four yearsHigh level of customer service experienceProven interpersonal, organizational, written, and verbal communication skills.Ability to work independently in a fast-paced, diverse, and multicultural environment.Demonstrated commitment to providing the highest level of satisfaction in client and staff service careSchedule :
Ability to see clients when needed. The ideal candidate is someone who has a part-time job and is able or willing to work casually
