Job description

In Your Role You Will

  • Travel to and assist persons in their private home or community settings
  • Implement person-centered plans designed to support to and achieve personal goals (including personal home living skills)
  • Instruct, guide, assist and model socially appropriate skills and behaviours.
  • Be aware of the individual’s physical, social and recreational environments, taking action to minimize risks and maximize participation / inclusion.
  • Assist individual in participating in a meaningful way in their personal life planning and .
  • Administer medical administration or first aid, according to prescribed procedures.
  • Participate in shift exchanges, staff communication and communication of issues or concerns.
  • Ensure comprehensive and accurate recording and reporting of information (e.g. daily logs, incidents reports)
  • Prior to confirmation of employment, all applicants must provide a copy of a current Criminal Record Check and / or Government Screening Letter. These checks vary depending on province and sector and will be communicated by the Program Hiring Team.

As An Ideal Candidate You Possess

  • 2 years experience working with persons with complex needs or acquired brain injury within the community.
  • Diploma in Disabilities Studies, Degree in related Social Sciences, Human Services, Special Education, etc.
  • Certified Brain Injury Specialist Certification training ( if necessary, employer will consider providing training)
  • Demonstrated skills in assisting individuals in anger management, daily living, self help, etc.
  • Able to collect information; prepare maintain, edit, and organize correspondence, Individual service plans, and progress reports; maintain files according to agency, ministry and accreditation standards.
  • Self-directed, requiring minimal supervision and cooperate with interdisciplinary team to solve problems, resolve conflicts and make decisions.
  • Display sound judgment and best practice methodology in progressing towards goals and solving problems.
  • Committed to philosophies and principles of community based living for individuals in service.
  • Good computer skills are required as all files are electronic
  • CPR / First Aid ( if necessary, will consider providing training)
  • Non Violent Crisis Intervention training
  • Abuse Protocol training (if necessary, will consider providing training)
  • Internal applicants must successfully complete their probationary period prior to applying
