Nurse Practitioner – Community Health Care CHC (Full-Time, Permanent)
Location : Scarborough, ON
Hours : 35 hours per week
Compensation : $51.29–$72.65 / hour
Employment Type : Permanent, Full-Time
About the Role
We’re seeking a passionate Primary Health Care Nurse Practitioner to deliver comprehensive, community-based primary care to unattached children and families. You’ll work collaboratively within an interprofessional model to promote wellness, manage chronic conditions, and provide accessible, high-quality healthcare to those who need it most.
Key Responsibilities
- Provide holistic, family-centered primary care services.
- Assess, diagnose, and treat acute and chronic illnesses.
- Develop and implement client-centered treatment plans.
- Provide routine and catch-up immunizations per Ministry of Health schedules.
- Maintain accurate, confidential health records.
- Participate in interprofessional consultations, case conferences, and chart audits.
- Contribute to outreach and health promotion initiatives.
- Support quality improvement and best-practice activities.
- Conduct home visits as required.
Qualifications
Master’s degree with Primary Health Care Nurse Practitioner certification .Active registration with the College of Nurses of Ontario (Extended Class) .3–5 years’ NP experience in a primary or community health setting.Current CPR (HCP) certification.Strong interpersonal, organizational, and communication skills.Ability to work both independently and collaboratively in diverse settings.Experience with electronic health record systems (e.g., Nightingale on Demand).Valid Ontario driver’s license and access to a vehicle preferred.Why Join TAPP Workforce Solutions
