Nurse Practitioner – Community Health Care CHC (Full-Time, Permanent)

Location : Scarborough, ON

Hours : 35 hours per week

Compensation : $51.29–$72.65 / hour

Employment Type : Permanent, Full-Time

About the Role

We’re seeking a passionate Primary Health Care Nurse Practitioner to deliver comprehensive, community-based primary care to unattached children and families. You’ll work collaboratively within an interprofessional model to promote wellness, manage chronic conditions, and provide accessible, high-quality healthcare to those who need it most.

Key Responsibilities

• Provide holistic, family-centered primary care services.
• Assess, diagnose, and treat acute and chronic illnesses.
• Develop and implement client-centered treatment plans.
• Provide routine and catch-up immunizations per Ministry of Health schedules.
• Maintain accurate, confidential health records.
• Participate in interprofessional consultations, case conferences, and chart audits.
• Contribute to outreach and health promotion initiatives.
• Support quality improvement and best-practice activities.
• Conduct home visits as required.
