Job Description

As a Nurse Practitioner, you have the power to redefine how patients experience recovery and reintegration. Responsibilities

• Work autonomously to diagnose, order, and interpret diagnostic tests
• Prescribe pharmaceuticals and perform procedures within your scope of practice
• Partner with physicians and care teams to support safe transitions from hospital to home
• Collaborate closely with patients, families and an interdisciplinary team to build personalized care plans aligned with their goals
• Contribute to quality improvement and innovation in transitional health care
