Position Summary :
This position involves working as a member of an inter-professional team offering collaborative, community-based comprehensive support to children / youth in Windsor / Essex under 18 years of age who are experiencing emotional / behavioural / social / complex mental health and / or developmental needs and their families. The Mental Health Social Worker I assumes the role of Coordinated Intake Access Service Planning. This position helps children, youth and families’ access appropriate services and supports quickly and easily.
Qualifications : Responsibilities :
- As part of the intake process, the client’s level of need and urgency is assessed in order to determine the appropriate service required, establish priority for service(s) based on risk, and identify the need for crisis services, where necessary
- Preliminary service options are communicated to the child or youth and family at intake.
- If appropriate, referrals are made to the most appropriate local service providers (e.g., through access to core services or through collaboration with or redirection to other sectors that better match the needs of the child / youth)
- The coordinated access process supports system-level planning and integrated case management.
- Developing and facilitating coordination and collaboration among community agencies and partners
- Participate in and co-facilitate community case conferences, multidisciplinary team meetings, and other client-related meetings for children / youth
- Co-facilitate monthly Local Resolution Table meetings (case resolution process)
- Assist in the referral process for local and out of town placements
- Coordinating and completing complex special needs applications (when required)
- To attend community case conferences as required.
- To participate in regularly scheduled supervision and seek out supervision and consultation as needed to provide effective services to assigned children / youth and families.
- Responsible for the completion of relevant reports in accordance with established standards (e.g., progress notes, assessment reports, etc.)
- To promote constructive relationships amongst service providers and children, adolescents and families
- Attendance at scheduled Departmental and Service / Program meetings.
- Participates in staff in-service training and consultation with other staff.
- Fulfills other duties as assigned to ensure that services are being provided in an efficient and effective manner.
QUALIFICATIONS :
