ABI in the Streets - Community Facilitator - Kitchener, ON
Traverse Independence supports adults with acquired brain injuries (ABI), often who also have concurrent disorders such as mental health and / or addictions and who are some of our most vulnerable in the community. They are often homeless or precariously housed and challenged by all the social determinants of health. This position is integrated within the primary care teams and works alongside the outreach teams in Kitchener, ON to provide brain injury support. It is an exciting position where the successful candidate will make a difference in the lives of many!
It is an advantage to have experience in the field of addictions and / or mental health and some experience in behaviour management and brain injury, but as brain injury is a specialized field, training both internally and externally will be offered to the successful candidate. You must have proven organizational skills, interpersonal skills, strong communication skills (both written an oral), and the ability to work as a member of a multi-disciplinary team with a comprehensive understanding of Microsoft Office products including O365.
What we offer

Education : Community College Diploma in Social Services, Health Care, or related discipline.
Knowledge / Skills : Demonstrated experience in behaviour management skills & knowledge of community resources.
