Accountabilities :
- Work collaboratively with the Pharmacy Team to provide patientcentred care
- Provide Pharmaceutical care and be part of a multi disciplinary team including medication reconciliation upon admission and discharge therapeutic drug monitoring
- Provide drug information to physicians nurses and other clinicians to enhance patient care
- Verification of all medication orders for inpatients
- Provide education to patients to promote optimal drug use
- Mentor Pharmacy residents students and interns
- Participate on hospital committees as required
- Dispensary duties including medication order entry and medication profile review
- Perform and / or supervise in patient dispensing (which includes computerized order entry unit dose centralized IV admixture services TPN Chemotherapy and an understanding of automated systems) drug information and administrative activities as assigned
- Resolve daytoday clinical issues
- Assist with the implementation of pharmacy initiatives which improve patient care and other initiatives which improve the efficiency and effectiveness of the Pharmacy operations
- Other duties as assigned
Qualifications :
Currently licensed (or eligible) by the Ontario College of Pharmacists Part A of the registryRelevant clinical experience preferableCompletion of hospital pharmacy residency or equivalent experience an assetExcellent attendance punctuality and work recordDemonstrate initiative and a willingness to work collaboratively in a multidisciplinary team environmentMotivated selfdirected learnerStrong clinical assessment skills in identifying and resolving drugrelated problemsDemonstrated ability to apply clinical judgment in analyzing and solving clinical problemsMay be required to travel between and work at all Osler sitesExcellent oral interpersonal and written skillsExcellent computer skillsMust be able to meet the physical demands of the positionMust demonstrate Oslers Values of Respect Excellence Service Compassion Innovation and CollaborationAdditional Information :
