About CMHA Toronto

CMHA Toronto is the city’s leading community mental health agency. We provide accessible, quality care for Toronto residents through programs, research and advocacy that create belonging and hope. At CMHA Toronto, we take a client-first, recovery-oriented approach to supporting people and their families when they experience mental health challenges or live with mental illness. By integrating health and social care, we provide services that aid in long-term wellness including housing, employment, community connections and so much more. Our goal? Help everyone thrive in the community.

Responsibilities

The Supportive Housing Worker (SHW) provides direct, hands-on support to clients living with mental health challenges who may also be experiencing housing instability or other barriers. The SHW assists clients with a range of daily living activities and tenancy supports, working collaboratively to promote recovery, wellness, and community integration.

  • Conducting housing and needs assessments to determine eligibility and readiness.
  • Supporting clients in developing and maintaining life skills (e.g., hygiene, budgeting, medication management, cooking, cleaning).
  • Providing coaching, mentoring, and orientation to residents to promote successful tenancy and daily routines.
  • Supporting clients in understanding their rights and responsibilities under the Residential Tenancies Act.
  • Liaising with landlords, housing staff, and property managers to resolve tenancy concerns and ensure positive relationships.
  • Assisting with move-ins, move-outs, and other housing transitions, acting as an Agent of the Landlord when required.

Beyond direct client support, the SHW plays a key role in ensuring program accountability, safety, and quality service delivery. Maintaining accurate, timely documentation and responding proactively to client needs are essential aspects of this role, such as:

  • Conducting regular unit inspections to ensure health and safety conditions are being kept
  • Responding to crisis calls and urgent issues reported by building or program staff, ensuring appropriate interventions and follow-ups are provided.
  • Maintaining up-to-date and accurate case documentation within the program database, ensuring that client information, progress notes, and service records are consistently recorded.
  • Collaborating with colleagues and community partners to coordinate care, share information appropriately, and identify opportunities for service improvement.

Qualifications

The ideal candidate will have:

  • Degree/Diploma in Social Work, Social Services or related field in human services
  • Experience providing community based direct service to adults with mental illness
  • Knowledge of the Residential Tenancy Act and supportive housing (including knowledge of the Landlord and Tenant Board)
  • Knowledge of the criminal justice system, concurrent disorders and issues affecting the homeless
  • Knowledge of the Ontario Support Disability Program and Ontario Works
  • Familiarity with homeless outreach initiatives and the shelter system
  • Good written and verbal communication skills
  • Ability to work effectively in a team as well as independently
  • Ability to foster and maintain relationships with the corporate sector and community agencies
  • Excellent interpersonal and conflict resolution skills
  • Experience working with diverse communities
  • Required to have a valid Ontario Driver’s License, access to a vehicle and insurance coverage.
