Physician

Position Overview :

Sweet Grass Health Centre is seeking an innovative Family Physician for a full-time (35 hours / week) or part-time position. We serve the Indigenous community by providing culturally safe, comprehensive healthcare services—including traditional healing, primary care, health promotion, addictions counselling, mental wellness care, diabetes care, and youth empowerment—to urban Aboriginal populations in Cochrane, Kapuskasing, Hearst, and partner First Nations such as Taykwa Tagamou, Wahgoshig, and Constance Lake. This role is integral to our approved Indigenous Interprofessional Primary Care Team (IIPCT), and it emphasizes best practices in community health while integrating traditional teachings and cultural values.

Key Responsibilities :

• Conduct comprehensive clinical assessments, diagnose, and treat a broad spectrum of acute and chronic conditions.
• Prescribe treatments, order diagnostic tests, and monitor patient progress.
• Coordinate care and manage follow-up to ensure high-quality, continuous patient management.
• Provide culturally sensitive care and education to patients and their families, emphasizing the connectedness of emotional, spiritual, physical, and mental well-being.
• Work within a multidisciplinary team comprising Physicians, Nurse Practitioners, R.N. (E.C.)’s, a Diabetes Care Team, Mental Wellness and Addiction Supports, Traditional Healers, Elders, and community health supports in Advocacy, Health Promotion, and Education.
• Develop collaborative practices with team members and accept referrals and consultations from Nurse Practitioners, R.N. (E.C.)’s, and other allied health professionals.
• Maintain accurate and timely medical records in compliance with regulatory and institutional policies.
• Contribute to team meetings, quality improvement initiatives, and community outreach programs.
• Assist in policy development and planning of health promotion activities that integrate traditional healing methods and Aboriginal cultural values.
