ROLE PROFILE :

Position : RAI MDS Partner

Reports to : Manager Business & Health Systems

Location : Head Office, Work from Home, and Travel within Ontario

Key Responsibilities :

  • In collaboration with the Director of Quality, Executive Director and DOC, provide follow-up and support to RAI-Coordinators in order to ensure that RAI MDS care meets and / or exceeds the organization’s and provincial health authority requirements
  • Support the development and strengthen capacity of the RAI-MDS processes and evidenced based practices.
  • Lead care community and organization-wide improvement processes / projects as directed and in consultation with the Director of Quality and RAI-MDS Coordinators
  • Assist and coach care community RAI Coordinators and clinical leaders to improve processes, strategies and activities in relation to RAI-MDS processes through the provision of education and orientation.
  • In collaboration with the RAI Coordinators support the generation, interpretation, and response to RAI-MDS quality indicators outcome reports.
  • Conduct weekly, monthly and quarterly quality audits in the care communities to determine effectiveness of services provided and assist in setting directions for change.
  • Reviews business performance on a monthly basis via monthly audits and on a quarterly basis via data quality reports and collaborates with team members to identify RAI-MDS quality successes and opportunities for improvement.
  • Reviews all data quality audits with RAI Coordinators prior to quarterly data submission.
  • Shares monthly PCC data and quarterly CIHI data to RAI Coordinators and clinical leaders via monthly and quarterly reports
  • Completes quarterly reports, for examples : CMI value from the RWPD / CIHI reports.
  • Participates in the orientation of care community’s RAI Coordinators
  • Leads monthly RAI Coordinator meetings in order to foster positive relationships and supports joint problem-solving, provide education and align with strategic directions
  • Supports care communities with developing back-up RAI-MDS processes, for sustainability and covering during vacancies of RAI Coordinator roles
  • Provide subject-matter expertise for the development of required RAI MDS program, policies and procedures based on current evidence
  • Manages and audits the Integrated Assessment Record (IAR)(ON)
  • Participates on community, educational or long term care committees as required
  • Prepares documented site visit reviews and distributes as appropriate
  • Participates on Support Services Offices Committees as requested
  • Provides recommendations regarding training and education, which would be of benefit the care communities and the organization as a whole

Education and Experience

  • Holds a current certificate of competence from the College of Nurses of Ontario and / or Health care professional, preferably a minimum three years’ experience.
  • A minimum of two years’ experience of Long-Term care or equivalent
  • Proficiency with Word, email, internet use, and computerized documentation
  • Ability to write and analyze reports, initiate, monitor and evaluate Benchmarking projects.
  • Proficiency with the RAI-MDS process, quality indicators and case mix index (CMI); Extensive Knowledge of RAI MDS 2.0, and CIHI CCRS.
  • Excellent leadership, analytical, and communication skills
  • Knowledge of MOHLTC Program Standards
  • Ability to travel in Ontario – access to a reliable vehicle and valid G license is required (
