Job Description
As part of a multi-disciplinary team, the incumbent will provide clinical social work and discharge planning services to inpatients on the Complex Transitional Care Units. The social worker will also :
- Conduct comprehensive social work and psychosocial assessments on inpatients, using a variety of modalities and in accordance with professional standards, clinical policies and practices
- Coordinate, formulate / develop, document, and implement the care plan for hospital discharge and safe transition back to community or institutional setting
- Participate in team conferences, educational / interdisciplinary rounds, and educational rounds
- Intervene to alleviate high risk or vulnerable situations
- Provide support to patient / family to aid in adjustment related to health issues and resolve complex interpersonal concerns
- Provide education regarding the HCCA and / or Substitute Decisions Act
- Provide selected therapeutic interventions, supportive counseling, community outreach and advocacy.
- Provide guidance regarding finances, legal issues, workplace injuries or community referrals.
- Support visitors requiring hospitalization
- Participate in advanced care planning and goals of care discussions, through family meetings and mediated conversations

BSW or MSW required; current registration and membership in good standing with OCSWSSWLEAP (Learning Essentials Approaches to Palliative Care) training preferred or to be completed within 1 year of date of hireExperience providing scope of practice social work to patients within an acute care or post-acute hospital settingStrong knowledge of community resources and demonstrated ability to work collaboratively and effectively with community partners.Familiarity with Mental Health Act, Consent to Treatment Act, Substitute Decisions Act, and Healthcare Consent Act.Demonstrated ability to maintain an evidence-based practice approach.Demonstrated previous group experience with this populationDemonstrated commitment to professional developmentDemonstrated ability to document according to college standardsDemonstrated ability to function independently and be self-directed to think critically and problem solve effectivelyExperience in interviewing skills and ability to provide comprehensive and timely psycho-social assessments.Strong knowledge of the Public Guardian and Trustee Office and Consent and Capacity BoardExcellent employment and attendance recordAdditional Information
