JOB SUMMARY
As a member of the interdisciplinary EMP team, the Licensed Practical Nurse is responsible for providing quality patient care within the scope of professional LPN practice to patients and families throughout their life span.
The incumbent will be part of the EMP Enhanced Care Coordination in Schools (CCS) program and will provide support to the team in their region. The goal of this team is to ensure enhanced clinical support to students with complex needs in public schools by providing care coordination and supporting Education and Early Childhood Development (EECD) and integrated care teams in place.
KEY RESPONSIBILITIES
- Observe, report and document changes in children’s condition and pertinent information as per professional standards and organizational policies.
- Participate and collaborate in the development, revision and implementation of enhanced care plans.
- Participate in admission to EMP when required.
- Collect and label specimens for laboratory tests.
- Demonstrate proper techniques, apply and demonstrate devices and equipment to the integrated team (school) according to the LPN scope of practice.
- Monitor children to ensure appropriate care, as necessary.
- Assist with activities of daily living and demonstrate, as necessary.
- Provide basic teaching to the integrated team (school) and families and reinforce complex child teaching in collaboration with the registered nurse.
- Demonstrate and teach proper techniques for isolation procedures, as necessary.
- Perform specific advanced Licensed Practical Nurse competencies as per organizational policies, as necessary.
REQUIRED QUALIFICATIONS AND OTHER JOB REQUIREMENTS :
Successful completion of an accredited school of Licensed Practical NursesActive registration with ANBLPN.Minimum of three years recent medical / surgical, palliative care and wound care experience in an acute care setting.Demonstrated education and experience related to medication administration, phlebotomy and physical assessment.A good driving record with a valid New Brunswick Class 5 Driver's License;Have a current CPR Level C certification or be prepared to obtain within 3 months;Ability to work efficiently as a member of the team and strong interpersonal skills, good judgment and decision making skills;Support student placement initiatives;Able to work all shifts as assigned, maintaining a good attendance record;A high regard for confidentiality; andSubmit a valid and clean criminal record check.All interested applicants are encouraged to apply as an equivalent combination of education, training and experience may be considered.
