Care Partner

What You’ll Do
- Manage and coordinate individual care plans for aged and disabled participants.
- Assess participant needs and develop tailored support strategies.
- Collaborate with participants, their families, and service providers to ensure optimal care delivery.
- Monitor participant progress and adjust care plans as needed.
- Oversee participant and service budget monitoring.
- Promote independence and community engagement for our participants.
- Work closely with a supportive and professional team to deliver exceptional service.
What We’re Looking For
- Minimum of 2 years’ experience (within the past 10 years) working as part of a team responsible for community-based aged care and disability services.
- Demonstrated advanced working knowledge and experience of the Home and Community Care Programs.
- Experience in working with participants and carers from culturally diverse backgrounds and those with dementia, frail aged, disability and mental health.
- Proven ability in participant and service budget monitoring.
- Compassionate, respectful, and excellent communication skills.
- Demonstrated experience identifying and working effectively with complex consumer care needs including but not limited to social economic issues, mental health, substance and/or elderly abuse.
