Job Title : Case Manager (On-Site)

Location : Detroit, MI (On-Site)

Shift : 8 : 00 AM - 4 : 30 PM

Contract Duration : 6 months

Pay Rate : $50 / hr

Role Summary :

Our client is seeking six experienced Case Managers for a 6-month contract in Detroit. These roles are focused on mentoring new hires and leading in a team transitioning from integrated case management to a dyad model, where Social Workers (SWs) and RNs function at the top of their license. The ideal candidates will have strong leadership experience in acute care case management, with the ability to guide new team members through complex discharges and difficult conversations with physicians and families.

Required Qualifications :

Experience :

Minimum of 2 years of leadership in acute care case management

Inpatient hospital case management experience

Discharge planning and utilization management experience

Recent full-time acute care hospital experience

Skills :

Epic and Careport experience required

Ability to mentor and counsel new hires

Licensure :

RN with BSN, or LMSW with Master's degree

Desired Qualifications :

Personnel management experience within an acute care hospital setting is highly desirable

Experience with transitioning case management models preferred

Additional Notes :

Candidates must have recent acute care hospital experience during or after the height of COVID

Inpatient psych case management experience will not be a good fit

Only candidates who do not need to relocate to metro Detroit will be considered
