Background : Flemingdon Health Centre (FHC) is an engaged and involved member of some of Toronto’s most dynamic neighborhoods; Flemingdon Park, Fairview and Thorncliffe Park. FHC is a registered charity and an incorporated not-for-profit Community Health Centre (CHC), with a vision of Strong Healthy Communities. Across our three sites, we provide a range of health-related services based on the social determinants of health with extensive community engagement. We are primarily funded through the Ontario Health.

Position Summary :
The dietitian role is part of an inter-professional team, providing individual assessment and counselling, group education sessions, program design and delivery as well as community outreach. Please note that the successful candidate will be providing services at FHC’s Flemingdon and Fairview sites.
Duties & Responsibilities :
Individual Nutrition Counselling
- Assess client's nutrition status by gaining an understanding of food habits or preferences, socio-economic and cultural background, psychological factors and medical profile
- Develop, implement and evaluate individual nutrition care plans according to the parameters of client's health, literacy level, stages of change, culture and socio-economic factors and provide nutrition counseling to clients
- Ensure quality of care by maintaining up-to-date records, monitor clients on a regular basis, and participate in case conferences, consultations, team meetings, educational sessions and quality improvement initiatives with other members of the primary health care team as required
