Position Description
St. Joseph's Healthcare Hamilton is a teaching affiliate of McMaster University and a tertiary referral centre for Central South Ontario offering services at three main sites, including St. Joseph's Hospital (Charlton Campus), the Centre for Ambulatory Health Services (King Campus), and the Margaret and Charles Juravinski Centre for Integrated Healthcare (West 5th Campus).
Position Summary
The Nurse Practitioner (NP) is an autonomous health care provider who will be working collaboratively with the interprofessional Urgent Care Team to support Virtual Urgent Care. The NP will be applying an expanded range of theoretical, practical and evidence-informed therapeutics to patients. The NP will be authorized to perform extended health services which include comprehensive assessment, diagnosis, intervention, referral, management, and evaluation of patient care across the continuum of acute health services. Utilizing advanced nursing knowledge, skill, clinical reasoning, problem solving and critical thinking, all duties and responsibilities will follow the NP Standards of Practice set by the College of Nurses of Ontario, and Standards of Behaviour set by St. Joseph's Healthcare.
St Joseph's Hamilton is a busy academic teaching hospital that supports interprofessional learning for multiple health professional programs, including NP students, PA Students, medical students, nursing, and others.
The Virtual Urgent Care program will serve patients in Ontario West and providing same or next day virtual access to patients who either don’t have a Primary Care Physician (PCP) or can’t access PCP within the required timeframe. The model is NP led. The NP will be located on-site at the King Street Campus of SJHH.
Qualifications
- Regulated health professional registered with the College of Nurses of Ontario registration in the RN (EC) class with no restrictions on registration or professional practice hearings pending. Either as an NP-Adult or NP-PHC, with significant relevant clinical expertise
- Masters in Nursing or relevant healthcare field required
- Completion of NP-Adult or NP-PHC program at the graduate level (Master's or Doctoral), from an approved academic facility
- Completion of NP-PHC program (Baccalaureate level) prior to 2000 from an accredited university program and a Masters in Nursing (alternatively, Masters in relevant health care field)
- Clinical Faculty appointment or eligibility to pursue clinical faculty appointment at McMaster University School of Nursing or Graduate Studies
- CNA Certification in related specialty an asset
- 3-5 years documented experience and expertise in the specified clinical area of hire and evidence of experience or ability to work in clinical settings across the continuum of care
- BCLS certified required
- Previous program development experience preferred
- Member of RNAO, NPAO membership preferred
- Proven clinical, educator, administrative and research skills
- Strong organizational, analytical, conceptual and communication skills
- Demonstrated clinical excellence and expertise in the specified clinical service area of hire
- Demonstrated excellence in interpersonal skills
- Demonstrated proficiency in computer skills (e.g., HBO / Star, Portal, Clinical Connect, MS Office, Patient Link etc.)
- Proven success providing inspirational leadership in a dynamic team environment and ability to encourage innovation and creativity amongst team members
- Commitment to the mission, vision, goals and values of St. Joseph's Healthcare and its nursing philosophy
- Must have excellent attendance and performance record
