DUTIES AND RESPONSIBILITIES :
Reporting to the Patient Care Manager, Emergency Mental Health, The Addictions Nurse Practitioner (NP) will work collaboratively with the Mental Health and Addictions (MHA) Program and Emergency Department (ED) programs as well as broad stakeholder groups to support implementation of the Substance Use Pathway Pilot; a collaboration between Lakeridge Health and Ontario Health, Centre of Excellence. The NP will become part of the new Addiction Medicine Consultation Service at Lakeridge Health Oshawa site, providing Interdisciplinary care for patients in the ED presenting with substance use concerns.
The Addictions NP will provide assessment and consultation services to adults and youth with substance use concerns in collaboration with the Division Head for the Addictions Program, the Psychiatry team and ED Physicians. The NP will function to scope and with a high level of autonomy and expert skill in the consultation, and care of patients with substance use concerns. The NP will provide evidenced based care, leadership and promote excellence in patient / family centered nursing care through expert clinical practice, education, consultation, system navigation and capacity building for patients with substance use disorders (SUD).
The NP is responsible for : autonomously diagnosing, ordering and interpreting diagnostic tests; prescribing pharmaceuticals and performing specific procedures within the legislated scope of practice; providing direct patient care focusing on health promotion, client centred care, and maximizing clients' safety and function during their participation in program services; actively participating in regular team meetings and in discharge decisions, liaising with community based and hospital providers, advocating on behalf of patients and their families and participating in inter-professional patient and family conferences as needed.
The successful candidate will have opportunities to participate in professional development and continuing education to maintain clinical competence and to maintain optimal quality of care to clients.
QUALIFICATIONS :
- Current RN (EC) registration with the Ontario College of Nurses, with a Nurse Practitioner certificate in Acute Care or Primary Care;
- Masters of Nursing required;
- Member of the RNAO and the Nurse Practitioner's Association of Ontario;
- 2+ years experience in the area of care for substance use disorders and related prescribing required;
- Knowledge, skills and ability to prescribe and monitor Opioid Agonist therapies, anti-craving medications, and withdrawal management medications for substance use disorders
- Demonstrated effective communication and interpersonal skills to work effectively on an inter-professional team and with community partners;
- Demonstrated ability to exercise independent judgement and to assess situations and problems efficiently and effectively;
- Knowledge of METAPHI recommendations and resources, as well as Health Quality Ontario, Quality Standards for SUD
- Commitment to patient-centred and trauma informed practices with a collaborative approach to clinical work
- Ability to interpret, apply and communicate relevant policies and procedures
- Knowledge and proficiency in current evidence-based mental health care and practices with an emphasis on health promotion and risk reduction;
- Must have satisfactory performance record;
