THE POSITION :
- We are seeking a dedicated Nurse Practitioner to join our team at a premier senior care facility, a top-quality independent living and assisted living facility focused on delivering exceptional, patient-centered care. In this role, you will oversee the clients in our 40-bed Transitional Care Unit, working directly with Southlake Hospital to ensure seamless care transitions and optimal health outcomes for our patients
