Job Description

Site : Toronto General Hospital

Department : Transplant Outpatient Pharmacy

Work Model : On-Site

Reports to : Manager

Grade : D0 : 09

Hours : 37.5 hours a week

Salary : $47.09 to $64.50 hourly (To commensurate with experience and consistent with UHN compensation policy)

Shifts : Monday to Friday, varies between 8 : 00 - 19 : 00 with rotating Saturdays 8 : 30 - 4 : 30

Status : Temporary Full Time, 12 Months

Closing Date : February 14, 2025

Duties

• Review, verify, check and dispense all prescribed medication orders for appropriateness
• Identify, resolve and prevent drug related problems
• Monitor drug therapy for effectiveness, toxicity and adherence to evidence based medicine / guidelines
• Follow up on medication incidents and promote safe medication practices on the unit
• Respond to drug information questions
• Provide direct and / or functional supervision of technician staff
• Perform other duties consistent with the job classification, as required
