We are recruiting for a Family Home Support Worker   to provide direct support to and arrange services for individuals living in Family Home settings under the appropriate relevant legislation and Agency Standards of Practice and in accordance with the stated goals of the Agency. To promote the interests of the person with intellectual disabilities within the community, working with natural families to support the Family Home placement. This is a one (1) year contract position with possibility of extension.

Compensation package :

Work up to 37.5 hours per week;
Minimum starting rate of $35.61;
11 statutory holidays are observed , with public holiday pay;
4% vacation pay paid on each pay cheque;
Confidential Employer paid Employee Assistance Program for you and your family members;

Minimum Requirements for this position include :

Bachelor of Social Work or a B.A. in Social Science / Human Services with recent relevant experience or college degree in a related field of study with combined experience, as determine by the employer ;

Minimum of 3 years recent full time rel a t ed experience in the field of social services ;

Understanding of the particular needs of persons with an intellectual disability and their families;

Understating of the MCCSS Social Inclusion Act;

Special factors of this position :

Must have a valid Class G driver's License and own reliable transportation;

Must be prepared to work flexible hours as required , including evenings and weekends ;

Participate in ongoing upgrading and training as required ;

Demonstrated excellent communication, listening and interpersonal skills required ;

Duties and Responsibilities (including, but not limited to) :

Developing a collaborative relationship with people in the family home program and natural families to promote the success of the family home placement;

The provision of emotional support to individuals;

Advocacy on behalf of individuals in the family home program;

Participate as a member of the Family Home team, working in collaboration with the Family Home Social Worker;

Assisting the Manager in identifying needs and recommending ways to meet the needs as they pertain to the people supported in the family home program;

Goal setting and assessing the program as per the Standards of Practice of the program;

Establishing and maintaining an effective relationship with people supported, their natural families and the Family Home Social Worker ;

Monitoring the quality of support that the people supported are receiving by meeting with people receiving supports, individual support workers, respite providers and natural families as outline in the Family Home MCCSS Directives ;

Ensuring records of people supported are maintained in accordance with appropriate legislation and the Standards of Practice of the Agency and MCCSS Family Home Directives;

Maintaining liaison contact with schools, daytime and other community agencies and professionals involved with the person;

Facilitating the development of support plans for each person supported in the program as per Agency Standards of Practice; reviewing support plans with the Family Home Social Worker ;

Supporting individuals to maintain or increase independence in all areas of life

Supporting individuals with Passport planning and spending

Supporting Individuals to access services and supports in the community to enhance quality of life

Maintaining regular contact with natural families of the people supported and where appropriate , encouraging their involvement;

Assisting with the admission and discharge planning of people supported as assigned by the Manager;
