Job descriptionMaintain accurate records and documentation. Work towards independent living outcomes for service users. Provide crisis intervention and empathetic support to clients. Monitor and evaluate clients’ progress, adjusting treatment plans as necessary. Develop tailored intervention plans and set measurable goals.
