Your opportunity :
The Accessibility Services Facilitator will triage and act as an initial point of contact for students, primarily for students accessing the Accessibility Services and Health & Wellness Complex Mental Health and ADHD Pathway. The incumbent will coordinate the connections and referrals to other central services within the University (i.e. Health & Wellness Centre) or to outside community health care agencies or individuals. This incumbent will support specialized initiatives that focus on the needs of more students with more complex needs where integrated care and collaboration with key partners is required. In addition, the incumbent will be responsible for supporting data collection and documentation for the purposes of student planning and program evaluation as well as creating programs and workshops using : a strong equity framework and analysis of the barriers for students with disabilities face. The incumbent liaises with community groups, on campus groups which utilize critical perspectives on disability within their work.
Your responsibilities will include :
- Providing case management support to students requiring the coordination of multiple services, particularly those with complex mental health and neurodevelopmental conditions.
- Providing pro-active support to individuals in distress when required.
- Probing for informationfrom students with personal and / or academic difficulties to ensure students are matched to appropriate referrals and resources to meet their needs.
- Liaising and collaborating with health care professionals to facilitate integrated case planning,effective communication and planning to facilitate the management of student cases.
- Interpreting and applying policies and procedures when making independent decisions.
- Planning and implementing components of curricular and / or co-curricular programs and activities.
- Building and strengthening relationships with partners of strategic importance.
- Facilitating educational workshops and seminars.
