Unit Summary
The McMaster Family Health Team (FHT) is sponsored by the academic Department of Family Medicine of McMaster
University and is located in two teaching units. McMaster Family Practice and Stonechurch Family Health Centre are
operated as a joint venture of HHS and the University. Their purpose is to train medical clerks and residents in Family
Medicine as well as other future family health team providers to provide primary care and to conduct research in the field
of family medicine. Stonechurch Family Health Centre (SFHC) is located in the rapidly growing southern part of Hamilton
Mountain and has a practice population of approximately 18,000 patients. McMaster Family Practice (MFP) is located
in the west end of Hamilton and has a practice population of approximately 16,000 patients. Patients come from all age
groups and all socio-economic backgrounds. Each teaching unit is led by a Unit Director. Primary care is delivered by
multidisciplinary teams of family physicians and their resident physicians, various allied health professionals and their
students, with support from clinical, technical, and administrative personnel. The hallmarks of the McMaster Family Health
Team are quality patient care, quality education and quality team work.
Position Summary
The Primary Health Care Nurse Practitioner (PHCNP) will be a Registered Nurse with an Extended Class license, be
registered with the College of Nurses of Ontario, and will work within the scope of practice of the RN-EC provided by
the College. Collaborating with interdisciplinary team members, and using a patient-centered model of care, the PCNP
is responsible to : provide direct patient care, including appropriate health prevention services, wellness programs,
and coordination of related community and other resources for the primary care patient population; tutor, mentor, and
supervise family medicine residents, nursing and other health professional students as appropriate, and participate
in continuing education activities to integrate the role of the PCNP in primary care and academic settings; initiate,
participate, and / or lead programs, presentations, publications, evaluations and other system changes to continue the
enhancement of interdisciplinary primary care teams.
Schedule Work Hours
Mondays to Fridays; days (some evening hours are required)
Qualifications
1. Certificate of Registration in the Extended Class from the College of Nurses of Ontario, and eligible to practice as a RNEC
2. Current Heart and Stroke Foundation course in CPR or equivalent
3. 3 to 5 years experience in a busy family practice (primary care) environment
4. Proven interpersonal, team, and communication skills
