Position Information:
Behavioural Supports Ontario (BSO) is a provincial initiative aimed at redesigning the system of care for older adults with complex and responsive behaviours associated with cognitive impairments, mental health and addictions, dementia, or other neurological conditions living in Long Term Care Homes and community settings. This position entails review, assessment, consultation and intervention for clients who are experiencing responsive behaviors/personal expressions and have been referred to the Oxford BSO (Behavior Supports Ontario) program.
The RN will work both independently and collaboratively as a participating member of the interdisciplinary care team within hospital and community sectors supporting older adults, their families and caregivers when concerns for responsive expressions are present.
Essential Job Functions:
•Conduct comprehensive medical, psychosocial and behavioral healthcare assessments as per BSO model of care requirements for referred clients.
• Provide direct clinical interventions, recommendations, modeling and staff education for the individuals, families and organizations served by the BSO program utilizing acceptable methods of service/work delivery.
• Maintain caseload as assigned by program director/designate.
• Maintain timely and accurate statistical records of workload activities, program reports, SERS etc. for BSO program, Mental Health Services (WH) and Ministry reporting purposes.
QUALIFICATIONS:
• Current registered member in good standing with the College of Nurses of Ontario
• Minimum 5 – 7 years Mental Health/Medical/Surgical/Complex Continuing Care inpatient experience.
• A minimum of 5 years’ experience working in the seniors’ services and programs within the community, Long Term Care facilities or Hospital settings (Mental Health& Addictions/Medical/Surgical/Complex Continuing Care inpatient experience)
• Certificates in Gerontology and Mental Health would be an asset
• P.I.E.C.E.S, Crisis Prevention, Gentle Persuasive Approaches (GPA), Montessori Methods Training, Dementability Certification OR other related training
• Current N.V.C.I.
• Completion of Understanding the 3D’s Delirium, Dementia & Depression (RNAO online workshop)
• Conflict resolution & crucial conversations training
• Current basic CPR
• Understanding and effective utilization of both pharmacological and non-pharmacological approaches to care when managing dementia patients & supporting families/caregivers.
• Understanding care transition pathways for older adults/families within the current healthcare climate and knowledge of the LTC application process.
• Able to work collaboratively with community partners, physicians, Hospital, Retirement & LTC sectors.
• Familiar / knowledgeable with the Long-Term Care Act.
• Familiar / knowledgeable with the Mental Health Act.
• Familiar / knowledgeable with Health Care Consent Act.
• Familiar / knowledgeable with privacy legislation.
• Satisfactory performance appraisals and attendance in current position if internal.
• Satisfactory attendance if internal.

Salary Range: $41.15/hr - $58.98/hr
