- Travel to and assist persons in their private home or community settings
- Implement person-centered plans designed to support to and achieve personal goals (including personal home living skills)
- Instruct, guide, assist and model socially appropriate skills and behaviours.
- Be aware of the individual’s physical, social and recreational environments, taking action to minimize risks and maximize participation / inclusion.
- Assist individual in participating in a meaningful way in their personal life planning and .
- Administer medical administration or first aid, according to prescribed procedures.
- Participate in shift exchanges, staff communication and communication of issues or concerns.
- Ensure comprehensive and accurate recording and reporting of information (e.g. daily logs, incidents reports)
- Prior to confirmation of employment, all applicants must provide a copy of a current Criminal Record Check and / or Government Screening Letter. These checks vary depending on province and sector and will be communicated by the Program Hiring Team.
