Job Posting : Practice NP primary care in a community setting, with a supportive team, and great work environment. There are opportunities for directly attaching patients as MRP, or shared-care with an existing primary care provider. Work at a clinical teaching site for the University of Toronto Temerty Faculty of Medicine.

Roles and Responsibilities :

• Manage chronic diseases in primary care.
• Order and interpret diagnostic imaging reports within the scope of RN, Extended Class.
• Perform physical exams, PAP test, well baby visits, immunizations, diabetes care, preventative health visits.
• Counselling for health promotion, prevention and disease management strategies for patients and families.
• Fill out various medical, workplace, government forms for patients.
• Documentation for medical, statistical, and administrative purposes.
• Review and manage results and consultation reports.
• Navigate health system for patients and families.

$55-$65 / hr, Health and Dental benefits

Hours : 24 - 40 / week
