Overview
We are seeking several Nurse Practitioners to work as members of the interprofessional primary care team, which includes physicians, nurse practitioners, nurses, administrative staff, and allied health professionals.
The NP operates as the Most Responsible Provider and will serve clients in collaboration with the interprofessional team, in walk-in and as rostered patients, provide coverage for clients of other providers when on leave, support same day / urgent appointment requests, participate in interdisciplinary case reviews, and support clients in accessing services available across the centre.
Qualifications :
- Baccalaureate in Nursing and Diploma / Master of Nursing (Nurse Practitioner)
- Registered with the College of Nurses of Ontario Extended class
- Minimum two years experience as a nurse practitioner in a community or primary care setting; Experience in mental health is an asset
- Strong clinical skills and diagnostic abilities : Performs medical examinations and evaluations, diagnoses, treatment, follow-up, consultation and health education
- Previous experience working with culturally diverse and marginalized populations
- Knowledge and training in trauma-informed care, psychotherapy modalities are an asset
- Knowledge and understanding of the Community Health Centre model of health and well-being
- Ability to work independently and be self-directed
- Effective organizational, critical thinking, problem-solving and decision-making skills
- Excellent communication skills and ability to adapt communication styles to meet the needs of clients, health team members and community partners
- Fluency in English is required. French and other languages is an asset
- Proficiency in the use of computers and various software applications
- Ability to work flexible hours, including occasional evenings and weekends
- Police Record Check for the Vulnerable Sector required
