We recognize that we work in our Residents' home, and we are there to support them and make sure they get the support they need.
Position Summary :
We are seeking a Nurse Practitioner to assess, diagnose, order, interpret medical tests, prescribe medications, and collaborate in the care of our Residents.
Key Responsibilities :
- Perform assessment, diagnosis, treatment, health education / counselling and other clinical activities according to the CNO standards of practice for NP's
- Contribute to chart documentation, participate in chart reviews and case conferences
- Facilitate continuity of care by working collaboratively with professional healthcare teams including primary care providers
- Provide primary or shared medical care, consultation and / or follow up to residents and their families
- Develop solutions that provide exceptional care to our residents as a leadership role within the home
