Position Title : Nurse Practitioner
Job Summary
We are looking for a Nurse Practitioner (NP) to join our client's team, a leading healthcare provider in the Durham region. As a member of an interdisciplinary team, the Nurse Practitioner (NP) will deliver comprehensive, client-centered primary health care, with a focus on accessibility, health promotion, illness prevention, and continuity of care for community members within high-priority populations. The NP will provide primary care assessments related to screening, prevention, treatment and referrals to community programs.
Responsibilities
- Deliver compassionate, client-centered primary care that respects each individual’s perspective, involving them as active partners in their healthcare. Recognize and address the impact of social factors on health and advocate for clients' overall well-being.
- Engage fully within a collaborative, interdisciplinary team, actively seeking and providing consultations to enhance patient outcomes.
- Perform thorough clinical assessments and collect detailed health information, particularly for clients with complex needs.
- Offer nursing services that addresses the biopsychosocial requirements of clients, including those with multiple comorbidities, mental health conditions, and / or substance use disorders.
- Provide holistic nursing services that consider clients' physical, psychological, and social needs, especially for those with chronic health conditions, mental health concerns, and / or substance use issues.
- Focus care efforts on priority groups, including newcomers and African, Caribbean, and Black (ACB) communities.
- Conduct intake assessments for new clients, ensuring seamless access to primary care Services.
- Improve service accessibility and navigation for clients through direct care, referrals, and resource guidance.
- Record and maintain clear, accurate, and timely client information and third-party documentation using the EMR system.
- Uphold safety and quality standards through evidence-based practices, focusing on teamwork, infection prevention, medication safety, proper equipment use, and risk management.
- Contribute to developing, implementing, and assessing medical directives, policies, best practices, and clinical guidelines.
- Attend agency training sessions and meetings as needed to stay informed and aligned with team goals.
- Offer guidance, supervision, and training to nursing students when necessary.

Employment Status : Temporary Full-Time Contract
Work Hours : 35 Hours per week (one evening per week )
Salary Range : $66- $69 hourly(commensurate on skills and experience, plus participation in Group Benefits & HOOPP.)
Job Types : Full-time, Fixed term contract
Contract length : 6 months
Pay : $121,119.00-$125,916.00 per year
