Mental Health Registered Practical Nurse

Reports To : Regional Director of Care Experience

Location : In-person in community or facility

Position Type : Full time, Permanent

Job Summary :

As Mental Health Registered Practical Nurse you will be responsible for conducting patients’ needs assessments and delivering nursing care to the patients, while providing person and family-centred care in community and / or facility settings, throughout the use of knowledge, skills, compassion, respect, and dignity.

Qualifications :

  • A regulated health professional RPN in good standing with CNO
  • Preferably 2-3 years of experience in mental health nursing
  • Completion of ASIST (Applied Suicide Intervention Skills Training) is an asset
  • Knowledge of mental illness, suicidal behaviors, crisis interventions, and current mental health practices and treatments
  • Working knowledge of the Mental Health Act, Health Care Consent Act and Substitute Decisions Act
  • Strong time management skills, confidentiality and professionalism
  • Excellent communicator, able to foster relationships with Home Care Staff and clients
  • Ability to work independently and work effectively within a team environment
  • Interest in continuous learning – high energy and focused
  • Current CPR-C / BLS certification
  • Police Criminal Record Check with Vulnerable Sector Screening
  • 2 Steps of Tuberculosis (TB) Test and Immunizations ( up to date)
  • Valid drivers license is an asset

Responsibilities :

  • Compassion, integrity, and quality care are all expectations of our community culture.
  • Assess or arrange for the initial and ongoing assessments and care plans of clients requiring unlicensed care and service as appropriate.
  • Visit patients in their homes for one-on-one visits
  • Assess patients’ conditions and chart observations, and perform evaluation tasks including vital signs and medication review
  • Educate patients and their families on proper home health care strategies and procedures, and make recommendations for devices or tools that might improve the patient’s quality of life
  • Provide effective communication to patients / clients, their family members, team members, and other health care professionals
  • Ensure that all clients concerns and complaints are addressed and documented in a timely manner.
  • Demonstrate knowledge of Standards of Practice guidelines of Provincial regulating body.
  • Maintain confidentiality of client, HCA and corporate information and discuss only with appropriate staff.
  • Participate in proactive Health & Safety activities while performing all duties.
  • Notify immediate Supervisor of any Health Concerns.

  • Complete all other tasks as required.
  • Visit patients in community / facility for one-on-one visits
  • Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients
  • Assess patients’ conditions and chart observations, and perform evaluation tasks including vital signs and medication review
  • Administer medication as prescribed by the patient’s physician
  • Educate patients and their families on proper home health care strategies and procedures, and make recommendations for devices or tools that might improve the patient’s quality of life
  • Provide effective communication to patients / clients, their family members, team members, and other health care professionals
  • Maintain confidentiality of client, PSW, and corporate information and discuss only with appropriate Bloom staff.
  • Monitoring of technology platform to ensure Care Pro's are on time, at their locations, and are actively and correctly utilizing their applications.
  • Provide prompt and courteous response to all customer inquiries and requirements and prepare required documentation
  • Create and update procedures on tasks relating to the duties of this position as requested.
  • Enforce and adhere to the codes of the company's Mission, Code of Conduct and policies both to their letter and intent
  • Perform other duties, as required.
  • Work Conditions :

  • Work requires travel within community or facility
  • May involve long hours, including evenings and weekends, to meet organizational needs.
  • On call after hours as needed
  • Physical Requirements :

  • Standing for extended periods involved
  • Lifting, pushing, turning involved
  • May repeat the same movement
  • Pay : $38.00-$42.00 per hour
