Job Overview

Reporting to the Associate Director of Care (ADoC) of the assigned Resident Home Area (RHA) the Registered Practical Nurse (RPN) serves Kensington Health’s LTC home (i.e. The Gardens at Kensington Health) to support and advance quality of care, safety and the experience of the people we serve.

This role will be guided by the legislation and regulations governing LTC in Ontario, the policies and procedures and best practices in accordance with the Standards of Practice of the College of Nurses of Ontario.

Responsibilities

  • Assumes responsibility for medication administration, narcotic control, removal of discontinued, unused or expired medications, application of treatments and dressings, transcription of on call and attending physician’s, orders, and personal care assessment as needed.

  • Completes assessments for new resident admission/transfers /readmissions/discharges.

  • Assesses the residents’ care needs and in collaboration with the resident, family and interdisciplinary care team develops and/or update resident care plans and communicates the changes to the team.

  • Communicates care changes with resident, family and personal care team (e.g., changes in health condition, changes in medications, etc.).

  • Communicates with and supports residents and families in their involvement and participation in resident’ s care.

  • Seeks assistance from the assigned Registered Nurse (RN) in completing clinical assessments for residents who present as unstable and/or present with a change in health condition (e.g. clinical deterioration, palliative and/or end of life).

  • Seeks assistance from the assigned RN with resident emergencies and /or other emergencies as they arise.

  • Documents in the health record, as per the organization's policies, procedures and the College of Nurses of Ontario's standards of practices.

  • Promotes the Resident’s Bill of Rights.

  • Promotes person-centered practices and approaches in all resident encounters and decisions.

  • Promotes an environment in which the health care team works co-operatively and supportively.

  • Participates in the Long-Term Care Home’s programs (e.g. Occupational Healt and Safety Program, Quality Improvement, Fire Safety, WHMIS).

  • Participates in required training and education and engages in quality improvement activities as needed.

  • In collaboration with the assigned RN serves as a clinical resource in identifying and solving resident care problems.

  • Responsible for working safely in compliance with accepted safe work practices, procedures and legislated health and safety standards. (i.e. Use of appropriate equipment, protective devices and personal protective equipment as required, reports hazards, accidents and the absence of or defect in any protective device.

  • Ensures clinical equipment and supplies are well maintained and provided input on equipment and repair needs as indicated.

Qualifications

Required Skills and Abilities:

  • Ability in any other language is an asset.

Required Knowledge and Experience:

  • Minimum of one year of experience in geriatric, rehabilitation or long-term care preferred.

Required Professional Designation/Certificate:

  • Current licensure with the College of Nurses of Ontario and in good standing.

  • Current Medication Certificate which meets the College of Nurses Standards of Practice.

  • Member of the Registered Practical Nurses Association of Ontario and/or the Gerontological Nursing Association of Ontario preferred.

  • Certification in Gerontology is desirable.

  • Medical clearance declaring fitness to work.
