Position : Social Worker (Full Time, 8am-4pm)

Key Responsibilities

• Receive and respond appropriately to inquiries pertaining to Southbridge Roseview which relate to new resident placement, including conducting tours of the home with prospective residents;
• Conduct pre-admission procedures including : screening of applications; correspondence with applicants to acknowledge receipt of application and to provide information; pre-admission visits and assessments; communication with placement agencies; navigate the resident / family through the admission process including coordination with the LHIN;
• Promote and facilitate a resident-oriented environment, acting as a resident / family advocate. Coordinate and serve as the home's liaison person for Family Council. Lead the Customer Service program within the home;
• Coordinate, plan and participate in annual and new admission interdisciplinary team care conferences for residents;
• Plan, organize and carry out social work programs / services to meet the ongoing psychosocial needs of the residents and their families / advocates;
• Provide palliative care support to residents and families as needed;
• Examine and act on matters concerning possible financial or emotional abuse of a resident and work collaboratively with the interdisciplinary team to address the issue;
• Lead the ethics committee for the home and be available to staff for consultation regarding ethical dilemmas within the home, working collaboratively with staff to resolve these dilemmas;
• Participate on committees related to resident care and operation of the facility as required;
• Function as a member of the multi-disciplinary team in planning overall resident care;
• Provide educational and consultative services on social work related issues / topics through participation in staff orientation programs, staff development programs and educational seminars for staff, residents and families / advocates as required.
