Responsabilité générale

Sous le leadership de la directrice de l'équipe en santé mentale la travailleuse l’accueil services en santé mentale accomplit les tâches administratives générales reliées à l’accueil de la clientèle et au soutien des professionnels en santé mentale (SME / SMEJ / IPC)

Responsabilités spécifiques

  • Recevoir et trier les nouvelles références des médecins de famille / infirmier(ère)s praticien(ne) (externe / interne), des conseils scolaires, de Contact Niagara, ouvrir les dossiers clients dans le système de DME (dossiers médicaux électroniques) et tous autres documents internes pertinents;
  • Effectuer le premier appel téléphonique aux clients pour introduction au programme IPC / SMA / SMEJ, compléter le questionnaire initial pour SMA / IPC / SMEJ, vérifier leurs coordonnées, inscrire les informations de base dans le système DME, effectuer les saisies de données requises pour les dossiers électroniques, ajouter les clients sur la liste d’attente et / ou leur fournir les ressources appropriées et / ou faciliter les références vers d’autres services;
  • Gérer la liste d’attente pour service avec les travailleurs d’approche et / ou thérapeutes dans les trois programmes.
  • Assurer une communication continue avec les médecins de famille / infirmier(ère) praticien(ne), CN et conseils scolaire, fournir toute mise à jour importante ou autre ressource pertinente;
  • Accueillir la clientèle du Centre de santé, cliniques médicales ou partenaires communautaires par téléphone, répondre aux demandes de renseignements généraux, acheminer les appels aux intervenants ou prendre les messages;
  • Préparer les trousses d'information et formulaires d’enregistrement destinées aux clients;
  • Tenir à jour la liste des ressources disponibles à l'interne et dans la communauté;
  • Enregistrer les rendez-vous de la clientèle, tenir les dossiers de la clientèle à jour;
  • Recevoir, trier et diffuser le courrier / fax électroniques; assurer les envois postaux ordinaires ou recommandés aux clients;
  • Classer les documents et assurer annuellement l'archivage des dossiers inactifs selon la procédure en vigueur;
  • Maintenir un stock suffisant de fournitures de bureau ou objets promotionnels, en tenir l’inventaire et faire les commandes selon les procédures établies;
  • Fournir un soutien administratif a la directrice des programmes en santé mentale, aux travailleurs d’approches et thérapeutes en santé mentale aux équipes du centre de santé selon les directives de la directrice du secteur;
  • Participer et appuyer la coordonnatrice avec l’organisation des réunions du personnel et réunions de secteur;
  • Appuyer le travail de la secrétaire médicale et de la réceptionniste au besoin;
  • Mettre en pratique les valeurs et les croyances du CSC ;
  • Protéger sa propre santé et sécurité en respectant la loi et en adoptant les politiques et les méthodes de travail sécuritaires du Centre de santé ;
  • Toute autre tâche connexe jugée nécessaire par la directrice

Aptitude et expérience

  • Excellentes compétences en informatique et maîtrise des technologies bureautiques;
  • Excellente aptitude à la communication en français avec la clientèle de tous âges et toutes origines, excellente maîtrise de l'anglais;
  • Aptitude à travailler de façon autonome au sein d’une équipe;
  • Expérience minimum de 2 ans dans un environnement de service à la clientèle et de travail de bureau;
  • Savoir s’adapter, avoir le sens de l’organisation, être capable de gérer plusieurs demandes en même temps, et avoir un sens aigu de la résolution de problèmes;
  • Être posée et aimable dans ses relations avec le public;
  • Connaissances / expériences en santé mentale / médical est un atout.
  • Formation / Qualifications

  • Diplôme d’études secondaires
  • Formation en base de données et en secrétariat considérée comme un atout

    Intake Worker – Mental Health Services

    1 Full time Contract Position (April 2025 - June 2026)

    Work location : Welland

    Annual Salary : $44,400 - $53,968, excellent benefits

    General responsibility

    Under the leadership of the Mental Health Team Director, the Intake Worker - Mental Health Services performs general administrative tasks related to customer intake and support for mental health professionals (EMS / SMEJ / IPC).

    Specific Responsibilities

  • Receive and sort new referrals from family physicians / nurse practitioners (outpatient / inpatient), school boards, Contact Niagara, open client files in EMR system (electronic medical records) and all other relevant internal documents;
  • Make the initial phone call to clients to introduce them to the IPC / SMA / SMEJ program, complete the initial questionnaire for SMA / IPC / SMEJ, verify their contact information, enter basic information into the EMR system, perform data entry required for electronic records, add clients to the waiting list and / or provide them with appropriate resources and / or facilitate referrals to other services;
  • Manage the waiting list for service with outreach workers and / or therapists in the three programs.
  • Ensure ongoing communication with family physicians / nurse practitioners, CNs and school boards, providing any important updates or other relevant resources;
  • Greet clients of the Centre de santé, medical clinics or community partners by telephone, answering general inquiries, routing calls to workers or taking messages;
  • Prepare customer information kits and registration forms;
  • Maintain an up-to-date list of resources available in-house and in the community;
  • Record customer appointments, keep customer files up to date;
  • Receive, sort and distribute electronic mail / fax; ensure regular or registered mailings to customers;
  • File documents and ensure annual archiving of inactive files according to current procedure;
  • Maintain an adequate stock of office supplies and promotional items, maintain inventory and place orders according to established procedures;
  • Provide administrative support to the Mental Health Program Director, outreach workers and mental health therapists, and to the health center teams, as directed by the Area Director;
  • Participate in and support the Coordinator with the organization of staff and sector meetings;
  • Support the work of the medical secretary and receptionist as needed;
  • Put into practice the values and beliefs of the CHC;
  • Protect her own health and safety by respecting the law and adopting the policies and safe working methods of the Health Center;
  • Any other related task deemed necessary by the Director.
  • Aptitude And Experience

  • Excellent computer skills and mastery of office technologies;
  • Excellent communication skills in French with customers of all ages and backgrounds, excellent command of English;
  • Ability to work independently as part of a team;
  • Minimum 2 years' experience in a customer service and office environment;
  • Adaptability, organizational skills, ability to handle several requests at once, and strong problem-solving skills;
  • Positive and friendly manner in dealing with the public;
  • Knowledge / experience in mental health / medical is an asset.
  • Training / qualifications

  • High school diploma
  • Database and secretarial training considered an asset
