Seeking Contract, Temporary, or Staff NP

THE POSITION :

We are seeking a dedicated Nurse Practitioner to join our team at a premier senior care facility, a top-quality independent living and assisted living facility focused on delivering exceptional, patient-centered care. In this role, you will oversee the clients in our 40-bed Transitional Care Unit, working directly with Southlake Hospital to ensure seamless care transitions and optimal health outcomes for our patients

PRIMARY RESPONSIBILITIES :

Oversee and manage care for the 40 patients in our Transitional Care Unit, in collaboration with Southlake Hospital

Conduct comprehensive health assessments, diagnose conditions, and develop personalized care plans

Prescribe medications, treatments, and therapies as appropriate

Collaborate with multidisciplinary teams to ensure high-quality, holistic care for patients

Serve as a primary resource for health education and counseling for patients and their families

Ensure adherence to all regulatory health and safety standards in senior and transitional care

QUALIFICATIONS :

Current Nurse Practitioner certification and license in Canada

Proven experience in transitional or geriatric care, with a strong understanding of chronic disease management

Exceptional communication, leadership, and collaboration skills

Commitment to providing compassionate, high-quality care
