Position Summary
Are you interested in being part of a team that provides person and family centred care? Join the interprofessional team in Smithers and be a part of something exciting.
Within the context of a person and family-centered model of care and in accordance with Northern Health's vision for an idealized system of services built on a foundation of Primary Health Care, the Primary Care Dietitian works collaboratively with interprofessional teams to develop a plan of care and deliver health services coordinated through the Primary Care Home. The Dietitian practices in accordance with the professional standards and code of ethics as outlined by the College of Health and Care Professionals of British Columbia.
The Dietitian provides competent, safe, and ethical care for people and their families in settings such as physician practices, homes, facilities, and communities, and applies the principles of evidence-informed practice to achieve effective outcomes related to their participation in valued activities. Through clinical assessment, treatment planning, recommended interventions, and follow up, the Dietitian contributes to the fulfillment of the overall plan of care developed by the interprofessional team and informed by the person's personal goals for care.
The Dietitian will facilitate individuals, together with their families and support networks, to achieve and maintain optimal health by performing the duties and responsibilities outlined.
Primary Care Networks are partnerships between the Divisions of Family Practice representing BC's family physicians, the regional health authorities, the First Nations Health Authority (FNHA) and local community partners, along with other community providers such as nurse practitioners. All partners are working together in their local communities and collectively to redesign B.C.'s primary and community care system so that it works better for patients and health-care providers.
This position is part of the Bulkley Valley Witset Primary Care Network (PCN), a collaborative partnership of the Witset Nation, First Nations Health Authority.
Shift Rotation / Hours of work : Days, 08 : 30 to 16 : 30 (rotating)
Smithers Smithers is located in the picturesque Bulkley Valley surrounded by three mountain ranges which provide a backdrop for the town's Alpine setting. The main industries include forestry, tourism, transportation and agriculture. Smithers offers affordable housing and natural splendor that support a balanced lifestyle. The population of Smithers is approximately 6,000, with an additional 5,000 from the surrounding communities that access services. There are three elementary schools, one secondary school, one alternate format school for grades 9-12 and Coast Mountain College. Check out Smithers where there is always something new to experience.
