Job Summary Reporting to UHN@Home, the Home Dialysis PSW will assist patients undergoing home dialysis by delivering direct care and ensuring the patient’s comfort and safety throughout the dialysis treatment. The role also supports other duties as needed for non‑dialysis patients, including general patient care tasks.

Duties

Perform home dialysis for patients based on delegation from a nurse or other regulated health care provider.

Provide direct patient care and ensure comfort and safety during dialysis sessions.

Collaborate with an external vendor to manage supplies and inventory in the patient’s home.

Maintain documentation pre, intra and post dialysis to support operational reporting.

Assist non‑dialysis patients with activities of daily living such as grooming, dressing, bathing, meal planning and preparation, mobility and transfers, and light household duties including cleaning, laundry and washing dishes.

Perform basic patient care functions as part of the plan of care.

Report outcomes and elevate safety concerns as required.

Qualifications

High school diploma and at least 1‑year Personal Support Worker Certificate from a recognized college, or an equivalent Occupational Therapist Assistant or Physiotherapy Assistant Certificate.

Completion of a Basic Cardiac Life Support (BCLS) program.

2+ years of practical experience in direct patient care.

Ability to handle daily mild to moderate physical exertion (standing, walking, climbing, reaching, sitting).

Computer literacy and basic proficiency, with willingness to learn new technology.

Fluency in English (verbal and written).

Reliable vehicle and ability to travel to patient locations as required.

Valid driver’s licence and adequate insurance (personal automobile policy with a minimum 1 million‑dollar liability limit).

Professional Affiliations / Memberships

HSCPOA (Health and Supportive Care Providers Oversight Authority).
