South-East Ottawa Community Health Centre (SEOCHC) is a non-profit, community-governed organization that provides a range of multi-disciplinary services to clients, including primary care, health promotion, social services, community and resource development and advocacy to address the social determinants of health. Our services and programs are designed to meet the needs of our community.

SEOCHC is excited to launch our Preventative Care & Wellness Clinics for Priority Populations and we are looking for an exceptional Nurse Practitioner to join our team and support us in delivering accessible, equitable and high-quality care to priority populations in our community. At the heart of our mission is providing low or no-barrier access to primary care for unattached and poorly attached individuals in our catchment and beyond. The priority populations include the African, Caribbean, and Black (ACB) communities, other racialized communities, 2SLGBTQIA+ communities, newcomers, and other underserved groups. The Preventative Care & Wellness Clinics will provide locally informed, culturally responsive interventions and clinical care with a focus on preventative care including proven cancer and chronic disease screening models. We will be working with community stakeholders and partners in designing our service and we are looking for a Nurse Practitioner that has flexibility to work evenings and weekends in addition to regular workdays.

We are seeking a highly skilled and motivated Nurse Practitioner to provide high-quality comprehensive primary care to clients in a fast-paced dynamic environment as casual / on call basis in clinic . As a Nurse Practitioner, youll deliver compassionate, culturally responsive care to priority populations through both in-person and virtual visits. Working within an interprofessional team, youll address chronic and episodic health issues, provide short-term medical management, support client advocacy, and connect clients to community resources. Youll also consult with team members, contribute to program development, and help advance SEOCHCs mission and strategic priorities through leadership and collaboration.

Please note : This is a new initiative that is still in development and may go through iterative changes to ensure we are developing services which are locally driven and meet the needs of our clients and communities. Individuals with an interest in contributing to the development of this initiative and those that either identify or have worked with the priority populations highlighted above are encouraged to apply.

KEY RESPONSIBILITIES :

  • Offer culturally responsive trauma-informed care with a focus on safety and engagement for our priority populations
  • Perform comprehensive clinical assessments and therapeutic interventions to clients with complex health statuses, mental health diagnoses, while recognizing the significance of social determinants and associated health risks
  • Initiate clinical pathways and refer clients to additional community-based and specialized medical services as needed, working collaboratively within the team.
  • Advocate for and connect priority populations to community supports both internally and externally
  • Identify service gaps and support the development of interventions to support priority populations.
  • Participate in quality improvement initiatives and support the development of inclusive clinic protocols and workflows
  • Provide consultation to team members to support high-quality evidence-informed care
  • Complete timely and accurate documentation of all client care and third-party correspondence using our electronic health record system and digital health tools. Use only approved methods and practices for communicating client information with others in the circle of care.
  • Utilize virtual care when appropriate to assess, diagnose and manage health concerns
  • Ensure client care and caseload is managed to achieve the goals of the program

DESIRED QUALIFICATIONS :

  • Baccalaureate in Nursing and Diploma / Master of Nursing (Nurse Practitioner)
  • Registered with the College of Nurses of Ontario Extended class, no restrictions
  • Minimum 3 years of experience working in primary care / community health serving equity-deserving populations (ACB community, other racialized groups, 2SLGBTQ+ groups, etc.)
  • Experienced with providing culturally responsive trauma-informed care and integrating anti-oppressive care practices
  • Strong clinical skills and diagnostic abilities in complex medicine, psychiatry; Perform medical examinations and evaluations, diagnoses, treatments, follow-ups, consultation and health education
  • Ability to effectively assess and manage mental health crises in a professional and culturally sensitive manner
  • Strong critical thinking, problem-solving skills, organizational and interpersonal skills with an ability to work effectively within a multidisciplinary team environment
  • Ability to effectively assess and manage mental health crises in a professional and culturally responsive manner
  • Strong communication, interpersonal and conflict resolution skills
  • Advanced computer literacy including proficiency with Microsoft Office applications and electronic health record systems (Telus PSS) and other digital health tools
  • Ability to thrive in a busy and fast-paced environment
  • Strong understanding of social determinants of health and their impact on health outcomes
  • Knowledge and understanding of the Community Health Centre model of health and well-being
  • Strong communication, interpersonal, and conflict resolution skills
  • Vulnerable sector police check completed with the last 12 months
  • Certificate of Basic Life Support (BLS) for Healthcare Provider or equivalent such as Cardio-pulmonary Resuscitation / Automated External Defibrillator (CPR / AED) Level C
  • English, French, and additional languages (Somali, Arabic, Spanish, Nepali, Kirundi, Pashto, Tigrinya, and others) will be an asset.
  • Ability to work flexible hours including evenings and weekends
  • Salary Range : $61.73 to $72.64 commensurate with experience
